For plan participants, our approach is very different (and a whole lot better) than anything they’ve ever experienced in the past. Some important elements include:

• Single Point of Service — By contacting our dedicated pod of Care Coordinators via an 800-number or secure website, members can get help with questions related to benefits, claims, network providers as well as receive patient advocacy, care management and nurse coaching.
• Co-branded Materials — To plan participants, our program is co-branded with the employers name and the Coordinated Health/Care™ program. All printed materials and the plan participant website are co-branded with the employer’s logo. As a result, plan participants better connect health plan sponsorship with the organization as well as improve the connection with any internal health and wellness or benefits initiatives.
  
• New Identification Card — Each member will receive a new identification card that they show to their health care providers upon the effective date. It will include some basic information for both them and their providers.
• Enhanced Benefits and Support — Members have the opportunity for an enhanced benefit if they begin all health care events with a call or visit to a primary physician and then show their new identification card to them during their visit.
